BHSB investigates and responds to critical incidents that occur at public behavioral health programs in Baltimore City. A critical incident is any actual or alleged event or situation that involves, or creates a significant risk of, serious harm to physical or mental health, safety or well-being.
Reportable critical incidents include, but are not limited to:
- serious injury
- medical errors
- medical emergency
- a missing person
- a suicide attempt
Providers are expected to report critical incidents to BHSB promptly. To report a critical incident, complete the BHSB critical incident form and send it to firstname.lastname@example.org.
