BHSB investigates and responds to Critical Incidents that occur at public behavioral health programs in Baltimore City. A Critical Incident is any actual or alleged event or situation that involves, or creates a significant risk of, serious harm to physical or mental health, safety or well-being.
Reportable Critical Incidents include, but are not limited to:
- abuse
- neglect
- death
- serious injury
- assault
- medical errors
- medical emergency
- a missing person
- a suicide attempt
To report a critical incident:
Providers licensed under COMAR 10.63 are required to report critical incidents to the BHA Office of Licensing & Compliance.
Non-licensed sub-vendors complete the BHSB Critical Incident Form and email the completed form to critical.incident@bhsbaltimore.org.
