Baltimore Capitation Project Referral — The Baltimore City Capitation Program provides intensive community-based treatment services for individuals with a serious and persistent mental illness through treatment teams consisting of a psychiatrist, team leader, registered nurse, personal service coordinators and community support staff. There are two programs in Baltimore City: Chesapeake Connections and Creative Alternatives.
Residential Rehabilitation Programs (RRP) – The Baltimore City Residential Rehabilitation Program (RRP) provides supervised housing and assists adults who have a serious and persistent mental illness with acquiring daily living skills necessary to maintain housing in the community. RRP is classified in two levels, intensive or general, based on the level of supervision needed by an individual. To apply for the program, please click here.
Medication-Assistance-Application -Application for Financial Assistance to Purchase Medications – A limited amount of funding is available to assist Baltimore City residents purchase psychiatric medications.
